Dr. Thomas does sports physicals for the local high schools and colleges for the areas of La Mirada, Whittier, La Habra, Buena Park and Santa Fe Springs. Please bring your school form for us to fill out, there is no standard form.

Student-athletes are not allowed to participate in interscholastic athletics unless all of the appropriate forms are filled out and turned in.
Dr. Thomas is at 15763 E. Imperial Hwy., La Mirada (Off Santa Gertrudes) in the Green Hills Shopping Center close to Fresh & Easy and Stein Mart.
We are offering physical exams at a very low cost to students.
